BBC Sport’s Unsung Hero award

Do you know someone who volunteers their time to help other people participate in triathlon? Then now is the time to nominate them for the BBC’s Sports Unsung Hero award.

Since being launched in 2003, this special award recognises someone who is dedicating their life to promoting sport in their community, taking no reward from it other than the pleasure of helping others to take part and enjoy their sport.  It does not matter how high a level they are at or how many people they are helping, simply that their efforts deserve recognition. 
 
BBC Sports Unsung Heroes are chosen in every region of the UK throughout November, so there will be an award winner in the area of the country you live in. An overall BBC Sports Unsung Hero will then be chosen from all our regional winners and announced on the BBC’s Sports Personality of the Year programme in December, live on BBC ONE.  
 
To nominate someone, simply fill in this form, answering all questions in conjunction with the Terms and Conditions that are published on the BBC website at http://www.bbc.co.uk/sportsunsunghero.  Then sign it at the bottom, and return it to the address below by 11.59pm on Monday 29th October 2012.
